Does a Vol. Fire Rescue have to have an ambulance to be the only rescue provider in their area?

Not necessarily - depends on the regulations in the area. If there's nothing else in the area, whatever the volunteer rescue can provide is good. The way I understand things, there is a long history of private providers as well that provide ambulance services. There are regulations, state and federal, regarding existing services so that they meet standards, but no standards regarding how many public, private or volunteer emergency services are required for a given area and what they must provide.
